If you are new to serving at LifeHouse, you will need to complete the Team Volunteer Application Form by clicking the link below (sorry, there is no printable version).

In addition, you will need to read and be prepared to sign a copy of the following documents with your Leader/Pastor (printed copies available at the church office).
LifeKIDS & House Youth
If you will be serving in any of our Kids or Youth Teams (or on a team who has team members under 14) you will also need to read and sign the following document (with your Leader or Pastor) as well as obtain a Working With Children Check prior to serving.
